Medical Clearance

A comprehensive history and physical must be done within 30 days of your scheduled surgery.  If two surgical appointments are necessary both must be scheduled within that 30 day time-frame or a new history and physical will be required.

After your physician conducts your medical exam and completes the a comprehensive history and physical it must be sent to our office at least one week prior to your surgical date (fax # 973-325-6723).

If you must cancel a surgical appointment, please notify your surgeon as soon as possible.

Medication

Unless otherwise instructed by your physician, please continue to take all your prescribed medications as directed.
